Nuclear Regulatory Commission Daily Reports Offer Insight

The U.S. Nuclear Regulatory Commission Operations Center provided several current event notification reports on Wednesday as part of the agency’s document collection efforts. For the 24-hour period ending December 22, 2021, the NRC disclosed nine separate reports including:

Radiation therapy medical underdose with no unintended health affects

Lantheus Medical Imaging, Inc. – Pallisades Park, NJ (Event 55644)

Missing 117 mCi of Thalium-201 reported by Massachusetts Radiation Control Program

Shipment in transit during vehicle maintenance

Generator received at destination but thalium remains missing

Less Than Cat 3 Radiation Level – Unlikely to cause permanent injury

 

Agriculture Veterinary Diagnostic Lab – Auburn, AL (Event 55645)

Disposal of Varian electron capture detector by unauthorized/unlicensed company reported by Alabama Department of Public Health Office of Radiation Control

Less Than Cat 3 Radiation Level – Unlikely to cause permanent injury

 

Park Nicollette Methodist Hospital – St. Louis Park, MN (Event 55646)

A 298 I-125 localization seed was lost in pathology department after removal

ECS Southeast. – Mt. Juliet, TN (Event 55647)

Damaged Troxler gauge reported by Tennessee Division of Radiological Health

Ran over by dump truck at local construction site; not operational

Gauge contains a 40 mCi Am:Be-241 and a 8 mCi Cs-137 source

Corrective actions will be updated within 30 days

 

Environmental Protection Agency – Denver, CO (Event 55648)

Missing Ludlum Model 192 survey instrument containing 1 microCi Cs-137 source

Shipment scheduled for Sweetwater, TX; common carrier reported that incorrect shipping label placed on package and sent to unknown location; last location of survey meter was in Hutchins, TX

Less Than Cat 3 Radiation Level – Unlikely to cause permanent injury

 

Methodist Le Bonheur Healthcare – Memphis, TN (Event 55650)

An I-125 seed was lost during removal following operation. Seed thought to have been lost during implant with surgical drape; reported by Tennessee Division of Radiological Health

Leaking Radium-226 aircraft dial at Kirkland Air Force Base

Dosimetry for base personnel did not identify any unexpected exposure

Leu Conversion Fabrication & Scrap Recovery – Richland, WA (Event 55666)

Sealed acrylic rod containing 5 mCi of Cs-137 removed by Qal-Tek (waste broker) failed leak test; reported by Framatome ANP Richland

Washington State Department of Health and U.S. NRC Region IV notified
